The School Code of Illinois requires that proof of a physical examination be provided at registration for all students entering the 9th grade and for all students who are enrolling in Marengo Community High School for the first time. Out-of-state students enrolling in Marengo Community High School are required to have a current physical examination from an Illinois physician. Forms for this examination are available in the Student Services Office. To be valid, a licensed physician or physician’s assistant must have administered these examinations within one year of the entry date into the previously cited grades.
ALL MARENGO COMMUNITY HIGH SCHOOL STUDENTS GRADES 9-12 must furnish proof of having received the required number of MMR (measles, mumps, rubella), polio, diphtheria, pertussis, tetanus, DPT and chicken pox by October 15th.
Immunization data can be found here.
